Adult Day Program
The Kiwanis Adult Day
Program is a not-for-profit community based program supporting adults
and older adults with varying physical and/or cognitive impairments.
We develop and deliver this community service with an
appreciation for individuality, life stages and social influences.
The purpose of the Adult Day Program is to improve the overall
quality of life and well being of clients through the provision of
meaningful activities while in a safe and supportive environment.
We strive to help adults and seniors living in
Hamilton discover, build and
maintain their skills, abilities and independence through participation
in social, recreational, intellectual, physical and creative programs.
The Adult Day Program is intended to provide respite services for
caregivers while helping participants to maintain their abilities in
order to remain at home as long as possible and to prevent frequent
hospitalization.
What is an Adult Day
Program?
An Adult Day Program is funded by the
Ministry of Health and Long-Term Care to provide a regularly planned
“day out” for frail seniors and adults with various cognitive and/or
physical impairments living within the City of
Hamilton.
The program is designed to provide social, recreational,
intellectual and physical activities in a structured, supervised group
setting. Throughout the day,
clients are provided with refreshments, a hot nutritional meal,
companionship and help with personal care needs such as medication
reminders and toileting.
Who is Eligible to Attend?
Our program is funded to provide services to:
frail seniors and adults with Alzheimer's disease, Dementia, or another
degenerative cognitive impairment such as Stroke, Acquired Brain Injury,
Parkinson’s Disease, Multiple Sclerosis, etc...) who live in the City of
Hamilton. Clients
who attend the program may also be socially isolated in their homes or
have need for personal care or support, due to a medical condition such
as Diabetes or other conditions.
The program is designed to provide regularly planned respite and
is alternative support for families and caregivers.
How Can I Apply to Attend?
To be considered for the Kiwanis Adult Day Program, a
referral must be made through the Hamilton Community Care Access Centre
(CCAC) at 905-523-8600 or if you are currently receiving CCAC services,
please ask your Case Manager.
Anyone can make the referral including YOU, a family member, your
doctor, a therapist, a neighbour, etc...
Once the referral is made, CCAC will contact you to complete an
assessment in order to determine eligibility and suitability.
After your assessment is completed, your name will be added to
the Kiwanis Adult Day Program wait list.
We will contact you once a space becomes available in the
program.
What are the Hours of Operation?
The Kiwanis Adult Day Program is open Monday through to Thursday
from 8:30am to 4pm.
Attendance in the program may vary from 1 to 4 days per week.
This is determined in partnership with the CCAC and is based on
the availability within the program and your individual needs.
As your needs change, there are opportunities to increase and
decrease your participation in the program.
(Please note: The program is closed on all statutory holidays.)
What Activities are Offered?
The Adult Day Program provides a wide range of recreational,
social, physical, therapeutic and mentally stimulating activities based
on clients’ needs and interests.
Some activities include:
- Arts & Crafts
- Fitness & Physical Activities i.e.:
swimming, walking program, bowling, etc..
- Bi-monthly Intergenerational Programs
- Monthly Pet Therapy Visits
- Special events, entertainment and
outings
- Trivia & Other Table Games i.e.:
cards, bingo, scrabble, etc...
- Health & Wellness Promotion
- Foot care & Hairdressing (available
for an additional fee)
and … many more!
How Much Does the Program Cost?
The Kiwanis Adult Day Program charges a nominal fee of $15.00 per day in order
to help cover some of the food and activity costs.
Subsidization may be available for those who qualify.
Occasionally, additional fees may be
charged if you register for extra services (i.e.: Foot care,
Transportation) or for some special events / outings (i.e.: Holiday
Dinner, Sailing Trip).
Is Transportation Provided?
Our program has a special partnership with Red Cross in order to provide safe
and reliable transportation to and from your home for an additional cost of
$2.10 per trip. Otherwise, you are
welcome to arrange for your own method of transportation or by contacting DARTS
at 905-529-1212 to apply for specialized transportation services.
